Request for Information and Confidentiality Agreement
Please complete the form below if you wish to request access to our confidential data site (the “Data Site”) pursuant to the terms of the indenture governing our 8.000% Senior Secured Notes due 2030 and/or 11.500% Senior Notes due 2031 (collectively, the "Notes").
Any person who requests access to such information agrees: (a) except to the extent otherwise required by law, to keep confidential and not to disclose or reveal the information obtained pursuant to accessing the Data Site to any person other than those of its representatives: (i) who are actively and directly involved with such person’s evaluations of the Notes, and (ii) whom the undersigned will cause to observe the terms of this request for information and confidentiality agreement; and (b) not to use information acquired pursuant to accessing the Data Site for any purpose other than the undersigned’s evaluation of its investment in the Notes.
